Summary:
Working under a grant from The Duke Endowment, Community Care of Western North Carolina has worked to improve care transitions in Western North Carolina by strengthening existing post-acute community coalitions and creating them in communities without a similar forum. These efforts have coincided with a shifting healthcare landscape that provides challenges and opportunities for community groups interested in improving transitions of care.

Objectives:
Participants will be able to:
· Identify the stakeholders needed to build a successful post-acute care coalition as well as key partners for technical assistance
· Describe the stages of group development and the processes used to further group work
· Describe key interventions & strategies used by post-acute care coalitions in Western North Carolina
