Our Stakeholders

NC ACT Stakeholders are partner organizations whose work aligns with the vision and mission of NC ACT, are actively involved and engaged in the activities of NC ACT, are willing to commit some resources (people, venue, data, funding, IT marketing) toward care transitions to ensure NCACT is successful, and have submitted a NC ACT Care Transitions Organization Profile.

NC ACT hosts annual Stakeholder Meetings for networking, sharing, and disseminating care transitions effective practices.  See the Events page for additional information on when these meetings will be scheduled.

  • Regular NC ACT Listserv communications including news, resources, model practices, and upcoming collaborative and educational events;
  • Free Regional Stakeholder Meetings scheduled across the state, great opportunities for regional and community networking and collaboration;
