The NC Alliance for Effective Care Transitions (NC ACT) was organized to connect services and resources, identify best practices and needs, and educate the public, providers, and decision makers on person centered quality services and supports for optimal healthcare when North Carolinians transition from one healthcare setting to another.
NC ACT Needs your Feedback!
NCACT has had a successful 7 years of convening and sharing of best practices. The Duke Endowment has generously funded our efforts but now is moving in a different direction, and no longer will sustain continuing funding for initiatives.
As a result we want to take this time since NCACT is at the end of grant funding cycle, to gather insight from our members on the relevance of NCACT in achieving your goals.

2017 NC Care Transitions Summit
Thank you for joining us for our 6th Annual NC Care Transitions Summit! With almost 350 in attendance the day was a hit! Attendees enjoyed a day full of networking, panel discussions, and poster presentations. 26 Hospitals were in attendance in addition to representatives from NC DHHS (multiple divisions), various hospice and home care agencies, Area Agencies on Aging, senior resources, community care agencies, nursing and rehab centers.
